When you start working with a new company, the definition record for NL Transactions does not exist in the system and the default form is being used. The moment you define a form of your choice, a definition record is being created. If you decide to remove the Form Template from the definition afterwards, the definition itself will stay in the system and will require the form to be defined. To return to the initial setup, please open the Transaction form definition record, go to menu Record and press Delete. This way the whole definition record will be removed which means the system will use the default again.
